Hei,

onko kukaan tietoinen, miten refluksitaudin uusia hoitomuotoja on otettu käyttöön täällä Suomessa?

Ja näillä uusilla tarkoitan nyt esimerkiksi EndoStimiä, Linxiä ja EsophyX-menetelmää. Sulkijaan vaikuttavat lääkkeet taisi epäonnistua mm. pahojen keskushermosto-oireiden takia :/

Introduction: Proton Pump Inhibitors (PPI) has been widely used over the last 2 decades as the most preferred therapy for all GERD cases, even though it has been established that 30-40% of GERD cases do not respond to PPI?s because they have visceral hyperalgesia more than hyperacidity. Newer therapies to treat patients with Refractory GERD or NERD cases can go a long way in improving the Quality of life (QOL) of these patients. This review summarizes a new approach in managing GERD by focusing on esophageal mucosal defense & use of mucosal protective agent on NERD or refractory GERD cases. A combination of hyaluronic acid along with chondroitin sulfate dispersed in a bio adhesive carrier (Poloxamer 407) to form a macromolecular complex, which helps coat the esophageal mucosa and acts as a buffering agent to form a mechanical barrier against the reflux. The hyaluronic acid also promotes wound repair and regeneration while chondroitin sulphate has anti-inflammatory and mucosal protective activity. Supportive trials have confirmed their efficacy either as adjuvant with PPIs or standalone medication to treat GERD cases including NERD and refractory GERD cases.

Spotlight on the Linx? Reflux Management System for the treatment of gastroesophageal reflux disease: evidence and research

Background The initial approach to gastroesophageal reflux disease (GERD) management typically involves lifestyle modification and medical therapy utilizing acid reducing agents such as histamine blockers and proton pump inhibitors. In severe cases refractory to such treatments, surgical therapy may be indicated. The gold standard for surgical treatment of GERD is the laparoscopic Nissen fundoplication. In recent years, a new technique known as magnetic sphincter augmentation (MSA) has been developed using the Linx? Reflux Management System. This is an implantable ring of magnetic beads that is placed around the esophagus at the gastroesophageal junction to restore lower esophageal integrity. The aim of this review is to discuss the current literature regarding indications, surgical technique, efficacy, and complications of MSA using the Linx device. Methods A standardized literature search was performed yielding 367 abstracts. After elimination due to duplicates between databases and irrelevance, 96 articles remained. The information found to be significant and non-redundant was included in this review. Conclusion After several years of clinical application, the Linx device has been shown to not only be effective for the management of GERD but also be as effective as fundoplication. With respect to safety, the most common complication of MSA is dysphagia. This often resolved without intervention, but esophageal dilation or device explanation are occasionally necessary. Not fully appreciated in earlier reviews, erosion of the device into the esophagus appears to be the most significant complication of the device after extended follow-up. While very rare, the potentially severe consequences of this phenomenon suggest that the device should be used with some restraint and that patients should be made aware of this potential morbidity. Fortunately, in the few cases of device erosion described in the literature reviewed, the Linx device was easily and safely removed.

Niin joo, ja tähän voisi lisätä tuon IQoron. Tilasin tämän muutama kuukausi sitten Ruotsista. En osaa vielä sanoa, että toimiiko. Olo on mulla ainakin parempi kuin aikaisemmin, mutta en osaa sanoa vielä mistä johtuu.

Linx-menetelmässä vissiin sellainen pienehkö riski että se magneettinauha alkaa erodoimaan, eli rikkomaan, ruokatorvea : 1/1000 leikkausta oli eroosiotapausta yhdessä tutkimuksessa, 2/164 toisessa (aika suuri ero!), samalla jouduttiin poistamaan kyseinen vehje. Linx ilmeisesti myös aiheuttaa enemmän nielemisvaikeuksia kuin se perinteinen Nisseni. Toisaalta molemmat leikkausmetodit näyttävät poistavan oireita lähestulkoon yhtä tehokkaasti.

Vanha tutkimus, mutta ehkä tämä voidaan laskea uudeksi hoitomuodoksi.. Eli Brasiliassa tutkimusryhmä on vuonna 2012 julkaissut tutkimuksen alasulkijan, eli LESin, hoidosta osteopatialla (käytännössä manipulaatiota). Ei liene kuitenkaan yleistynyt hoitomuotona, en ainakaan löytänyt mitään uudempaa julkaisua aiheesta.

Melko tuore (heinäkuu 2018) LINX leikkauksen vertailu PPI-lääkitykseen 6 kuukauden seurannalla (12 kk tulokset ilmeisesti julkaistaan myöhemmin).

Arvaatte varmaan kumpi oli tehokkaampi.

No niin, löytyipäs uudempi tutkimus osteopatian/manipulaation mahdollisesta tehosta refluksin hoitoon, vuodelta 2017.

Norjalainen yhteistyö tällä kertaa.
